Congenital and Adult Cardiac Screening

Some clients and organizations have special requirements for a pet to be certified free of heart disease by a board certified cardiologist. In such cases, you may even work directly with a veterinary cardiologist to provide this service. Depending on the pet, organization specific requirements, and even breed, you may need this form to be filled out by a board certified veterinary cardiologist.  Please contact me if you have questions.
